摘要:
目的观察益气养阴活血方治疗早期糖尿病肾病(diabeticnephropathy,DN)的临床疗效。方法将120例患者按随机数字表法分为治疗组和对照组各60例(纳入统计治疗组56例,对照组57例),两组均常规用胰岛素或口服降糖药控制血糖,对照组予以盐酸贝纳普利(benazepri1hydroch1oridetab1ets,BHT)治疗,治疗组在对照组的基础上加用益气养阴活血方治疗。观察治疗前后空腹血糖(fastingplasmaglucose,FPG)、餐后2小时血糖、糖化血红蛋白、尿微量白蛋白排泄率(urinaryalbuminexcretionrate,UAER)、血清胆固醇(serumcholesterol,TC)、甘油三酯(triglyceride,TG)、血液流变学指标的变化。结果治疗组疗效优于对照组(P<0.05);治疗后2组FPG均有显著性下降(P<0.01),但两组间比较无显著性差异(P>0.05);两组UAER治疗后均有显著性下降(P<0.01),但治疗组下降更为明显,优于对照组(P<0.05);治疗组治疗后TC、TG均有显著性下降(P<0.01),而对照组TC、TG的下降则无显著差异(P>0.05);治疗组治疗后TC、TG的改善较对照组有显著性差异(P<0.01);两组血液流变学各项指标在治疗后均有显著性下降(P<0.01),但治疗组全血粘度低切及高切、红细胞聚集指数等方面的改善显著优于对照组(P<0.01)。结论常规治疗结合益气养阴活血方治疗早期糖尿病肾病,能改善患者的血液流变学指标,改善肾脏微循环,减少尿蛋白的排出,保护肾功能。
Abstract:
ObjectiveTo observe the clinical efficacy of Yiqi Yangyin Huoxue Decoction for treating the early diabetic nephropathy (DN). Methods120 patients were randomly but equally divided into treatment group and control group, each with 60 cases at the beginning (56 cases in treatment group and 57 cases in control group were finally included for further research), cases in both groups took insulin or oral hypoglycemic agents conventionally to control blood sugar, and cases in control group took benazepri1 hydroch1oride tab1ets (BHT), on the basis of the control group, cases in treatment group took Yiqi Yangyin Huoxue Decoction. Fasting plasma glucose (FPG), 2hour postprandial blood glucose, glycosylated hemoglobin, urinary albumin excretion rate (UAER), serum total cholesterol (TC), triglyceride (TG) and hemorheology changes were observed before and after the treatment. ResultsClinical efficacy of treatment group was better than control group (P＜005); FPG were significantly decreased in 2 groups (P＜001), but difference between the two groups showed no significance (P＞0.05); UAER was significantly decreased in 2 groups (P＜001), but it decreased more significantly in treatment group than control group (P＜005); TC and TG were significantly decreased in treatment group (P＜001), while TC and TG were not significantly decreased in control group (P＞005); TC and TG showed significant differences between treatment group and control group after treatment (P＜001); hemorheological parameters significantly decreased in two groups after treatment (P＜001), but improvements of whole blood viscosity low shear and high shear, erythrocyte aggregation index were significantly better than control group (P＜001). ConclusionThe conventional treatment with Yiqi Yangyin Huoxue Decoction can improve hemorheology, renal microcirculation, and reduce the discharge of urine protein, and protect renal function in early diabetic nephropathy patients.【
